This is my SAS/Graph version of the graph from p. 33 of the following publication:

This is a pretty standard "proc gbarline" chart, and then I annotate some 
text labels on it (to get them exactly where I want them).  I have to get 
a little tricky with the labels on the line, and calculate the y-position
as a % value, based on the data value and the endpoints of the axis.
